Our Clinical Model

Clinicians (physicians and psychologists) working on behalf of Solace Health Medical Group, Inc., perform an initial evaluation through taking the patient's history, assessing for additional diagnoses, and evaluating for suitability for treatment during the intake appointment. If the patient would benefit from Solace's services, the patient is paired with a Solace advocate, who works incident to the initiating clinician.

Our advocates are RNs, LCSWs, and other licensed clinical staff who further assess the patient for barriers to care and health literacy, create a personalized care plan, assist the patient with acute condition and chronic condition management, and identify solutions for health-related social needs that impact a patient's acute care utilization and ability to appropriately manage their medical or behavioral health conditions in the outpatient setting.
